Sports Physicals will be offered for the upcoming 2013-2014 school year on Wednesday, May 15th. Registration starts at 3:30pm and will be on a first come, first serve basis in the High School Gym of Rockford Christian High School. The sports physicals will be conducted from 4:00pm to 6:30pm by Accelerated Rehabilitation Centers and Dr. Milos of the Lundholm Surgical Group. All athletes are required to have a current sports physical and athletic emergency form on file with the athletic department prior to the start of practice with a sports team. Sports physicals conducted will be valid for 395 days. IHSA sports physical and athletic emergency forms are available at www.rockfordchristian.org under the athletics menu at forms and documents or can be picked up in the athletic department office. It is mandatory that student athletes have their parents signature on the IHSA pre-participation examination form with the student-athletes medical history for them to have a sports physical.
The cost of the sports physical will be $20.00. Payment can be made by cash or check. Checks can be made payable "Rockford Christian Schools"
Sports Medicine The athletic department has partnered with Accelerated Rehabilitation Centers and Lundholm Surgical Group to offer premiere services for the sports medicine program at Rockford Christian Schools. Through this partnership student athletes will have access to an athletic trainer through Accelerated Rehabilitation Centers for all practices and home games at Rockford Christian High School. In addition, Dr. Steven Milos will be the premiere team physician for the athletic department from Lundholm Surgical Group. It is the goal of the athletic department to offer a sports medicine program that will provide care and rehabilitation services to the student athlete benefiting them physically and spiritually while participating in the athletic programs at Rockford Christian Schools.
